National Diabetes Prevention Program Graphic

The Rx for the National Diabetes Prevention Program: Action Guide for Community Pharmacists Cdc-pdf[PDF – 12 MB] is designed to help community pharmacists and members of the pharmacy workforce reach people at high risk of developing type 2 diabetes who could benefit from the National Diabetes Prevention Program (National DPP) lifestyle change program and prevent new cases of type 2 diabetes among patients at high risk by taking steps to help expand the reach of the National DPP. The guide provides case studies of pharmacies that have already taken these steps, highlights various ways pharmacists can engage with the National DPP, and is framed around a “three tiers of engagement” (i.e. Promoting Awareness; Screening , Testing, and Referring; and Delivering the Program), to allow pharmacies to pick and choose which tiers to engage with based on their own resources and capacity.
